An appointment request is not informed consent for treatment.

Submitting an inquiry requests contact from the practice. It does not establish a treatment relationship, guarantee availability, confirm that a service is appropriate, or replace the informed-consent process.

If the practice and prospective patient decide to proceed, the responsible clinician will explain the service, provider status, supervision when applicable, fees, scheduling, privacy limits, telehealth requirements, risks, benefits, and alternatives before treatment or evaluation begins.
